I will throw out the name Thomas Daniel Weiskopf as the architect who passed too soon and call it Forest Dunes. Not sure about the hole (or even if my other two entries are correct).
It is not Forest Dunes, but I agree that Weiskopf passed too soon. As with Weiskopf, the architect of this course also passed too soon.
